CHW Cardiovascular Health Specialty Track
Learn the fundamentals of cardiovascular health, including risk factors, major cardiovascular diseases and conditions, treatment approaches, and lifestyle strategies for prevention and management. This program equips learners, especially Community Health Workers, with the knowledge and skills to support individuals and communities in reducing cardiovascular risk, promoting healthy behaviors, and managing chronic conditions. Specialty tracks like this are designed for already certified CHWs who have core competency skills and want to hone their expertise in a specific topic area. Learners can complete all units (1–4) as a full track or mix and match units to create a custom bundle that fits their needs. Contact us to design a pathway tailored for your specific learners.
Learning objectives: Define chronic conditions and explain their impact on individuals, families, and communities. Identify common chronic conditions, including hypertension, diabetes, prediabetes, asthma, and select cancers, along with their risk factors, prevention strategies, and management approaches. Describe the role of Community Health Workers (CHWs) in supporting clients with chronic conditions, including care coordination, communication, and resource navigation. Promote self-management strategies to help clients take an active role in managing their conditions. Recognize the social determinants of health that affect chronic disease prevention and control. Connect clients to community-based programs and healthcare resources for screening, diagnosis, and management.
